WHEN COVERAGE STARTS

For purposes of this section, the terms "newborn" child" and "adopted child" apply only to your children or children of your spouse.

Your coverage starts on your effective date. You should check with your Group or with us to be certain about your effective date. If you are a new member, please remember that you will be covered only for services provided or arranged by a Medical Group Physician. If you or any eligible dependents are confined to a hospital, skilled nursing facility or other institution on your effective date, you must notify us immediately so that we can transfer your care to a designated Hospital or skilled nursing facility and to a Medical Group Physician.

Whenever you add a new eligible dependent to your coverage, his/her coverage starts as follows:

Spouse - A new spouse may be enrolled by submitting a change of enrollment form to your Group within 31 days of marriage. Coverage starts on the first day of the month following our receipt of your enrollment form.

Newborn Child .- Newborn children are covered for 31 days from the date of birth. To continue coverage beyond the 31 days, you MUST enroll the child within the 31 days. However, remember that only services that we actually provide or arrange will be covered from date of birth.

Adopted Child - An eligible adopted child must be enrolled within 31 days from the date the child is placed in your custody. Coverage starts on the date of placement.

Others - Employees and dependents who are not enrolled when newly eligible must wait until the next open enrollment period to become members of Kaiser Permanente.

Your Group may have special eligibility rules or restrictions which affect this section. Please check with your Group if you have any questions.
